We are currently accepting new registrations to the practice.
You will need to complete in full a Registration Form and a New Patient Health Check Form – both need to be handed into Reception.
You will need to know your NHS number and you can get this from your current GP. Please can you also ask your current GP for a summary of your medical records – they will allow us to get you medical records / medications / repeat prescriptions set up.
In order to register please ask the receptionist for the relevant forms.
Please be aware that it may be some time before we have access to your old medical records once you register at the surgery which is why it is important for you to get a summary from your previous GP.
